3. Every purchase of an IVO SB151 Water Purifier Set earns one (1) raffle entry and will get the chance to win any of the following prizes:

MAJOR PRIZE:

One (1) winner of a Travel Voucher to Japan worth seventy five thousand pesos (PHP 75,000.00)

Travel Voucher is redeemable via a nominated third-party partner travel agency, which will handle all arrangements, including but not limited to VISA processing, airline tickets, transfers, and accommodations.
Travel Voucher is valid for one (1) year from date of issuance. Advance reservation is required. Booking and travel must be completed no later than the expiry date.
All travel expenses exceeding PHP 75,000 will be the responsibility of the individual prize winner and, where applicable, their nominated guests.

MINOR PRIZES:

Five (5) winners of IVO 5-Year Bundle which includes one (1) IVO SB151 Water Purifier Set and five (5) IVO C151 Value Pack (valued at PHP 18,480 each)
Five (5) winners of IVO C151 Value Pack (valued at PHP 3,300 each)

4. To join the raffle, customers must go to the raffle.ivo.ph website, completely fill out the online form with his/her complete name, complete address, contact number, email address, transaction or proof of purchase reference number, and upload a clear copy or screenshot of their valid proof of purchase / receipt / invoice / etc.

5. DEADLINE OF SUBMISSION OF ENTRIES: July 7, 2023

6. The raffle draw will be held at Moduvi, Inc. Head Office located at Unit 605 The Centerpoint Building, Garnet Road corner Doña Julia Vargas Avenue, Ortigas Center, Pasig City, seven (7) days after the end of the registration period, that is on July 14, 2023 at 3:00pm.

7. The raffle draw will be witnessed by representatives from the Department of Trade and Industry (DTI) and Moduvi, Inc.

8. Customers can win only one prize. Should a customer with multiple raffle entries be drawn more than once, then the prize with a higher value is automatically awarded to the customer. Another winner will then be drawn to receive the prize with the lesser value, if applicable.

9. Moduvi, Inc. reserves the right, in concurrence with DTI, to reject a participant if he/she is unable to satisfy the conditions set out in this promo mechanics.

10. IVO Product Prizes are not convertible to cash and cannot be exchanged for other items, while the Trip to Japan Travel Voucher Prize is transferable and may also be converted to CASH—net of applicable taxes and fees (where applicable).

11. The winner will be notified by phone and an official notification letter sent via registered mail and e-mail, based on the respective filled out form on the website. To claim the prize, winners must:

Submit the original official notification letter
Present two (2) valid government issued IDs
Sign the Release, Waiver, and Quit Claim Form
For provincial winners: Moduvi, Inc. representatives will directly coordinate and may require online submission of documents stated above. Moduvi, Inc representatives may also request for a scheduled online conference call / meeting and upon further verification, will ship the prizes directly to them.

12. All entries remain the sole property of Moduvi, Inc. with all participant information held within Moduvi, Inc.’s secured database of entries. Except where prohibited by law, participation in the Promotion constitutes each participant’s consent to Moduvi, Inc.’s use of their name, personal description, likeness and opinions for any promotional, marketing and publicity purposes in any media. The winner may be required to participate in publicity program which Moduvi, Inc. may organize in connection with the Promotion and if required to do so, such winner must attend such function. All Winners shall agree to release, indemnify and hold Moduvi, Inc. and partners harmless from any and all losses, damages, rights, claims and actions of any kind rising in connection with or as a result of participating in the Raffle or the Winner’s acceptance or use of any Prize.

13. The schedule and location of awarding shall be agreed upon by both a Moduvi, Inc. representative and the winners. By default, awarding of winners will be documented at Moduvi, Inc. Head Office located at Unit 605 The Centerpoint Building., Doña Julia Vargas Avenue corner Garnet Road, Ortigas Center, Pasig City, Metro Manila. For provincial winners, they may claim prize at the Authorized Store or Reseller where they made the winning transaction.

14. If the winner is unable to claim the prize him/herself, a representative must present the necessary documents:

(14.a) a letter of authorization duly signed by the winner, nominating the parent/ legal guardian, or representative—at least 18 years of age, residing in the Philippines, with a valid Philippine address—as the acknowledged recipient of the prize on the winners’ behalf
(14.b) Two (2) original government-issued IDs of the winner with picture and signature (acceptable/valid IDs are National ID, SSS, TIN, Passport, Postal ID, Voters ID, etc.)
(14. c) Two (2) original government-issued IDs of the representative/legal guardian.

15. Prizes not claimed after a sixty (60) day period will be forfeited and may be used as deemed necessary by Moduvi, Inc. upon approval of DTI.

16. Moduvi, Inc. reserves the right to verify the eligibility of the drawn entries (that is, the winner’s proof of purchase with the issuing store).

17. Employees of Moduvi, inc. its affiliates, suppliers, resellers, ambassadors, concessionaires, third party agents and their relatives up to the second degree of affinity or consanguinity are disqualified from joining the promo.
